Your Visit

When you make your appointment our staff will review the nature of your visit (new patient, new problem (existing patient), follow-up visit) and the possibility of your accessing on-line the documentation necessary. Bring the following with you for your appointment.

• Insurance Cards
• List of Medications/Allergies
• CD with radiographs/CTscan/MRI
• Medical Records from another provider who has treated you for this condition

Appointments 

• Arrive 15 minutes to complete paperwork
• If you need to cancel or reschedule, please call at least one business day prior

Payment

• Payment, including copays and deductible, are expected at the time of service
• Accepted forms of payment are cash, check or credit card

Insurance 

We participate in most insurance plans.  If your carrier is not on the list below, please contact us to verify.

• Workers Compensation
• Medicare
• Fidelis
• Excellus/BCBS

Disability and other work-related forms

• Please allow 3-5 business days for forms to be completed
• There may be a fee for some forms, you will be informed prior to completion if a fee will be assessed.

    Medical Records

• We will automatically send a copy of your visit to your PCP of record at the time of your visit
• For all other requests, please complete the medical record release form below

Emergencies 

• In any life-threatening emergency call 911
• For any musculoskeletal emergency go to the nearest affiliated hospital Emergency Room.

If you have downloaded one of the Intake Forms you can fill it in.  You can send it to us before your appointment by fax, or by scanning and then email to: office@tvsmallman.com. Or,  you can simply bring the completed form with you when you present in the office for your appointment, or you can send it to us by fax, or by scanning and then email to:
